Adjustments: In March 1967 (NCDOT), US 401's NB split in Raleigh was changed to use South St west to McDowell north instead of using Lenoir St.
In Nov 1967 (NCDOT), US 401 was placed on its Fayetteville bypass, replacing part of NC 59. The old route through Fayetteville became US 401 Business (some of Russell St and Robeson St have been removed as primary routing since).

About 1973 (approved by AASHO November 1970), US 401 was moved onto the new US 52 bypass around Darlington SC. Lamar Hwy became SC 34 CONN; Pearl St remained SC 34; Main St became US 52 Business.
About 1977, it appears that US 15-401 Bypass of Bennettsville SC became just US 15-401. There is no AASHTO documentation but that is when they did document doing the same thing in Hartsville SC. While US 15 through Bennettsville became US 15 Business, it is unclear if US 401 became US 401 Business as no SCDOT map shows it. There is also no AASHTO documentation of dropping US 15 Business when that happened in 1984 or 1985. Today the route is SC 385 and SC 385 CONN.

In April 1984 (NCDOT), US 401 was placed on the Raleigh Beltline around the east of Raleigh.
In July 1991 (NCDOT), US 401 was removed from the Raleigh Beltline and put back through Downtown Raleigh as it runs today.

In 1994, US 401 was removed from Raeford Rd at Lake Rim west of Fayetteville (now Old Raeford Rd and Raeford Rd cutoff by I-295).
In June 2001 (NCDOT), US 401 was extended north with US 1 to end at I-85 just short of the Virginia line.

US 401 #1: Appeared in the 1925 US Route plan, running from US 1 Sanford NC via Durham and Clarksville VA to US 1 South Hill VA.
In June 1926 (AASHO), US 401 was extended with US 1 through Sanford, then Carthage, Pinehurst, West End, Ellerbe, and Rockingham before ending at the South Carolina line (not sure where).
In September 1926 (AASHO), US 401 became US 15 instead. Today, Rockingham to Pinehurst is not in the US route system. Pinehurst to Clarksville VA is still US 15. Clarksville to South Hill is the US 58 corridor.
A couple maps show the 1925 routing of US 401. No map I've seen shows it south of Sanford.

US 401 #2: Appeared March 1927 (AASHO) as the first US Highway to be added after the initial Dec 1926 roll-out. It ran from US 15 (now US 58 Bus at 5th St) Clarksville VA to US 1 (now Goodes Ferry Rd) South Hill VA. This was a renumbering of part of US 15.
In 1927, US 1 was moved off SR 903 Goodes Ferry Rd and instead onto its current routing down to NC. The 1931 route log says US 401 still ended at South Hill so US 401 may have still ended at Goodes Ferry Rd. Either way, US 401 was removed from SR 651 Old Plank Rd between Lombardy Grove and South Hill.
In 1930, US 401 was placed on modern US 58 from Boydton to US 1 Lombary Grove. The old route used a road now abandoned save a small piece of SR 608 Ridge Rd that follwed the RR tracks closely (north side) from US 58 Bus Boydton until about the current US 58-VA 4 jct. US 401 followed SR 674 Old Plank Rd to Baskerville; SR 669 Baskerville Rd; SR 673/SR 663 Busy Bee Rd; SR 645 Gordon Lake Rd; SR 664 Union Level Rd to Lombardy Grove.
Also by March 1930, US 401 was extended west over VA 44 from Clarksville to end at US 501 South Boston.

In 1931, US 401 was moved to US 58's routing from South Boston to today's SR 750 west of Clarksville. US 401 was removed from SR 742 Alphonse Dairy Rd; SR 601 Stevens Rd at Omega; lengthy SR 601 Buckshoal Rd/Lovetown Rd and SR 732 Tabernacle Rd at Buffalo Springs.
In July 1931 (AASHO), US 401 was renumbered as part of US 58. It remains US 58 except: SR 750, 8th St, then US 58 Business through Clarksville; some underwater routing on the east end of where the US 58 Business bridge is; SR 783 through Finchley; SR 756, SR 707, then US 58 Business through Boydton.

US 401 #3: In March 1932, AASHO responded to South Carolina's request to have a new US route from Raleigh NC to Walterboro SC or Savannah GA by proposing this route be an extension of US 501. In May 1932 South Carolina requested this corridor be numbered as US 301 instead, but in June 1932 AASHO said it would be US 401 and run from US 17 (now SC 63) Walterboro SC to US 70 (now Lenoir St) Raleigh NC.
US 401 replaced SC 30 from Walterboro through Santee, Sumter, Hartsville and Bennettsville to North Carolina above McColl SC.

US 401 was added to NC 24 through Laurinburg to Fayetteville and NC 21 from Fayetteville to Raleigh.
In July 1934, AASHO informed South Carolina of its intention to replace US 401 in its entirety there with US 15. South Carolina replied that they would rather US 15 head more eastward to replace US 701 to Georgetown SC.
In September 1934 (AASHO), agreement was reached to replace US 401 with US 15 in South Carolina.
In February 1935, the Sumter SC Board of Trade informed AASHO that South Carolina had posted US 15 signs, but that North Carolina was refusing to route US 15 over former US 401's route there. Indeed the 1935 NC Official map draft showed US 401 in SC as US 15, but left US 15 running to Rockingham and US 401 as if nothing changed.
In March 1935, AASHO informed South Carolina that North Carolina had now told them directly that they wanted US 15 to replace US 501 in NC and that US 401 would be unchanged. North Carolina's highway department had changfed officials and apparently had not put these changes in writing and therefore the current regime would not implement them due to pushback from certain communities. AASHO told SC that they would have to accept North Carolina's refusal and hoped SC had not yet posted US 15.
In August 1935, AASHO was informed that North Carolina was going to replace US 401 with US 15-E (while replacing US 501 with US 15-W).
In October 1935, AASHO was informed that North Carolina was replacing US 401 with US 15 ALT.
In December 1935, AASHO was informed that South Carolina had responded to North Carolina's posting of US 15 ALT to double post US 15 in its entirety from Walterboro to North Carolina as a temporary measure until the whole thing could be sorted. AASHO confirmed with South Carolina that they did this, and while not enthused, AASHO did concede this was how it was going to be.
In effect, US 401 was renumbered in its entirety as US 15 ALT.

Today this is US 15 from Walterboro to Society Hill SC except: removed causeway (but some bridges still stand) immediately west of the now-closed US 15-301 Lake Marion bridge; S-42-5 Main St through central Sumter; S-43-325 at Whites Mill Pond; US 15 Bus through Hartsville
This is again US 401 from Society Hill to Raleigh, except: abandoned causeway/Pee Dee River bridge/S-35-167 Kolbs Tomb Rd for 2 miles/2.5 miles abandoned road back to US 15; SC 385 and SC 385 Conn through Bennettsville; US 401 Bus through Laurinburg; abandoned causeway immediately west at the Lumber River; US 401 Bus through Raeford; Raeford/cutoff by I-295/Old Raeford Rd; US 401 Bus into Fayetteville, but stayed on Raeford Rd all the way in to Broadford, then north to Hat St then east into central Fayetteville, then north at Ramsey St back to US 401 Bus; NC 217/Old US 15A loop; Chalybeate Rd through Chalybeate Springs; in Raleigh it used Saunders to Lenoir.
