US 401 ALT #1: Appeared June 1956 when US 15 ALT became US 401. The 1956 Official map already shows Laurinburg NC as bypassed by US 15 ALT, with the old Main St route through town still shown as primary. It is unknown what the town route was designated but it became US 401 ALT.
About 1960, US 401 ALT was redesignated as US 401 Business.

US 401 ALT #2: Appeared sometime after 1957. The Raeford NC bypass was shown on the 1956 Official but the old US 15 ALT route using Harris/Main/Central through town was not shown as primary.
About 1960, US 401 ALT was redesignated as US 401 Business.
US 401 ALT did not appear on maps but it was explicitly mention in 1960 correspondence with AASHO about all the unapproved changes North Carolina had been making in the late 1950s.
