Creation: Created May 1930 (AASHO), running from US 221-321 Boone NC to US 70-170 (now Walker at Aycock) Greensboro NC. This was an addition to NC 60. It is possible US 421 continued to the center of town at Market/Elm intersection.

Adjustments: In October 1930 (AASHO), North Carolina passed a resolution to get US 421 extended west from Boone NC to Johnson City via US 221 to Blowing Rock and Linville, NC 181 to Newland, NC 194 to Elk Park, and US 19-E into Tennessee. In May 1931 they submitted a request to AASHO to do this, then later in the same document requested that US 421 be renumbered as US 44 or US 72, plus also requesting US 421 be extended southeast to Wilmington.
In June 1931, US 421 was extended over NC 60 to Wilmington, but AASHO denied extending it west to Johnson City TN. In their acknowledgement of the May requests, AASHO immediately rejected the renumbering part of the request.
In June 1932 (AASHO), US 421 was extended north, replacing US 321 to Bristol VA, then replacing US 411 west to end at US 25-E Cumberland Gap VA (now 0.20 miles west of the parking lot for the Cumberland Gap area).

In a September 1933 letter to AASHO, North Carolina expressed its desire to extend US 421 across Tennessee, given that Virginia changed US 421 to US 58.
In 1933, US 421 was removed from Walker Ave in Greensboro to use Market St all the way west from downtown. Walker Ave may have briefly been US 421-A for a time after this.

About 1935, US 421 was extended south with NC 40 to Ft. Fisher.

Between 1931-36, US 421 was removed from Archie Carroll Rd east of Boone NC.

In March 1944 (AASHO), US 421 was again truncated to Bristol, leaving behind US 58. In Virginia's request VDOH expresses surprise that US 421 was co-routed with US 58 to Cumberland Gap. This is odd because state official maps from 1935-44 explicitly show this as does the 1941 route log (but not 1933 or 1935). However, the March 1935 publication of AASHO US Routes does have US 421 going to Cumberland Gap. My speculation is that AASHO did not correctly interpret Virginia's request regarding US 58 which was to cancel US 421 in Virginia. As this 1938 photo shows, US 421 was not posted at Cumberland Gap.

Between 1945-49, US 421 was given one-way splits in Dunn NC - NB used the original routing of Broad St while SB used Cumberland St.
In July 1949 (AASHO), US 421 was extended north to the state line on the Ohio River bridge between Milton KY and Madison IN. US 421 was added to US 58 from Bristol to Dot VA; replaced VA/KY 66 to Harlan KY; replaced KY 257 to Hyden; added to KY 80 to Manchester; replaced KY 21 to Bighill; replaced KY 169 to Kingston; added to US 25 to Lexington; replaced KY 50 to Frankfort; replaced KY 37 to the Ohio River.
About 1949, US 421 was rerouted in Greensboro into a wide split routing: NB left Asheboro Rd using S. Benbow Rd north to Market St west into downtown. SB left Market using Greene St south to McGee St east to Elm south to Asheboro Rd as before.

Also in 1949, US 421 was removed from Old Greensboro Rd east of Winston-Salem NC and moved to New Greensboro Rd (no longer continuous) southewest to 5th St.
Also around 1949, US 421 was removed from Arnold Way/Pemberton Rd plus Old Shady Rd/abandoned alignment/Baines Dr in Holston Valley TN; US 421 was also given its modern South Holston Lake routing, leaving behind an extended TN 44, Knob Park Rd, underwater routing southeast, Jacob Park Rd where it leaves an arm of the lake, a little Denton Valley Rd, and abandoned routing s-se back to current US 421 at Jacks Corner.
In September 1950 (AASHO), US 421 was extended north to end at US 12 Michigan City IN. It replaced IN 29 to Boyleston; added to IN 28 to Frankfort IN; added to IN 39 to Monticello IN; added to US 24 to Reynolds IN; replaced IN 43 to Michigan City.

Between 1950-52, US 421 was taken off SR 1340 Old Bridge Site Rd in Castle Hayne NC.
In February 1951 (CTB), US 421 was removed from SR 706 Main St in Pennington Gap VA, plus a bit of VA 70 (now US 58 ALT) and put on its current routing north of US 58 ALT.
In May 1952 (CTB), US 421 was placed on split alignments in Bristol VA/TN. NB continued to use State Ave all the way. SB left State Ave on 9th (now Volunteer Pkwy) south to Shelby east to 4th (now Martin Luther King Jr. Blvd) north back to State to continue over to Pennsylvania Ave as before. So some of US 421 SB dipped back into Tennessee.
In 1952, US 421 was given its bypass of Clinton NC using Southwest Blvd, a bit of current NC 24 and what is now the US 421-701 freeway. The old route through downtown Clinton became US 421-A.
Also about 1952, US 421 was extended south from Fort Fisher to the Fort Fisher Ferry area at Federal Point. Also, US 421 was shifted westward at the Fort Fisher area to escape the impending ocean creep. The southernmost segment of Battle Acre Rd was part of the original road but the rest up to about Surfrider Ct is now in open water. See US Ends.com for more information.
Between 1950-53, US 421 was moved from Greensboro Ave to 2nd Ave north of downtown Siler City NC, which was given grade separation from US 64.
In June 1954 (AASHO), US 421 was rerouted at NC 210 south of Wards Corner to avoid Castle Hayne and instead used its current routing south to today's US 74/NC 133. US 421 then cut southeast to cross the river with US 17-74-76 onto Parsley and then to 3rd St as it had historically run. The old route to Castle Hayne became NC 210 and US 117 remained in place south to Wilmington.

In 1955, US 421 received a bypass around the south of Kernersville NC, using new construction at what is now the closed Exit 19 of I-40 Business (now US 421) and following this west to Exit 10 (now Exit 228) and returning to Old Greensboro Road. The road through Kernersville remained mostly NC 66. Today this is Mountain St.

In 1956, US 421 was rebuilt east of current TN 167 and removed from Rhea Rd (no longer connects to)/Old Prison Camp Rd; Dowell Loop; P Snyder Rd; Drakes Creek Loop; Poplar Ridge Rd
Between 1954-57, US 421 was removed from Kitty Fork Rd northwest of Clinton NC, as well as the Old US 421 loop near Harnett County line.
Also between 1954-57, US 421 received its bypass of Buies Creek NC, leaving behind SR 2054 Marshbanks St and NC 27 (now just Leslie Campbell Ave).
Also between 1954-57, US 421 was removed from SR 2195 Thrift/Murchison Rds in the Gulf NC area in southeastern Chatham County.

In 1957, US 421's wide split routing in Greensboro was eliminated. US 421 followed Market St east from downtown to O Henry Blvd south to what is now I-40 west briefly to Liberty Rd.
Also about 1957, US 421 was rerouted at Yadkinville NC to head south with US 601 then west into Wilkes County as newly constructed highway (Rocky Branch Rd). The old road became SR 1314 Old US 421.
Also in 1957, US 421 was removed from North Wilkesboro and Millers Creek NC by following NC 268 west to Curtis Bridge Rd out to current US 421. The old route remained NC 18-268 to North Wilkesboro; NC 16 to Millers Creek; SR 1304 west back out to US 421. Today this is Wilkesboro Blvd; 10th St; D St; Old Boone Trail
In 1958, US 421 was placed on Cumberland Ave both directions through Dunn NC, then also was placed on today's E Jackson Blvd bypassing nearby Erwin. The old route became SR 1718 Denim Dr and a little NC 82-217.
Also about 1958, US 421 was removed from Curator Ct/abandoned/Benson Ave just west of Frankfort.
In October 1959, AASHO approved adding US 421 to I-74 between I-465 Indianapolis and Greensburg IN.
In 1960, US 421 was added to I-74 between IN 9 Shelbyville IN and Acton Rd. This left behind Michagan Rd from IN 9 to London Rd; Michigan Rd through Pleasant View; Southeastern Ave east of Acton Rd. In between these 3 segments I-74 was built on top of US 421.

In 1961, US 421 was removed from surface streets in Winston-Salem NC and moved onto then-I-40 from 5th St westward, then onto its own new freeway west to Yadkin County. 5th St through W-S to Broad became US 158 Business; Broad/7th/West End Blvd/Reynolda Rd became NC 67. Yadkinville Rd west of NC 67 became SR 1525.

Also in 1961, US 421 was placed on new construction from the Yadkin River to US 601, leaving behind a bit of US 601 plus SR 1605 Yadkin Rd.
Also in 1961, US 421 was placed on new construction from the Sugar Grove NC area to the TN state line at Zionville, leaving behind a bit of US 321 and SR 1233 through Amantha and Mabel.
Also in 1961, US 421 was placed on the US 25 bypass routing around the east and north of Lexington that was now completed to US 421 nortwest of town. The route through Lexington became US 421 Business.
Also in 1961, US 421 was added to I-74 between IN 244 and IN 9 around Shelbyville. This left behind Michigan Rd; IN 44 Broadway St; IN 9 Harrison St/Riley Hwy. Additionally, US 421 was added to I-74 between Acton Rd and Southeastern Ave Indianapolis. This left behind Southeastern Ave.
In 1962, US 421 was removed from SR 1291 Old US 421 through Luart, Mamers, and Arlington NC and placed instead on its current routing west of Lillington.
About 1963, US 421 was removed from abandoned/Turner Station Rd to make room for I-75 in the MM 101 area; also was rebuilt on new alignment from KY 418 to about Old Toods Ln in Lexington KY. Surviving segments are a parking access rd just north of KY 418 and some of Richmond Service Rd on the east side.
In 1965, US 421 was given its bypass of Harrells NC leaving behind NC 41-411 and Wildcat Rd.
Also about 1965, US 421 was rerouted onto Wilkinson Blvd around the north of Frankfort KY. The old route remained US 60-460 into downtown, US 127 (now KY 2261) on High St; Broadway St.

In June 1966 (NCDOT), US 421 was removed from a portion of Lee Ave and Main St in southern Sanford NC and placed on mostly new construction (now Horner Blvd). The Main St portion became an extended NC 78.
In October 1966 (CTB), US 23-58-421 was placed on a newly constructed bypass of Gate City VA, leaving behind US 23-58-421 Business.
In December 1966 (CTB), US 421 was rerouted in Bristol VA by being removed from State St into central Bristol. Instead it replaced VA 76 along Goodson St to Mary St, then replaced VA 76Y along Mary St and Piedmont St, then was added to US 11-19 south along Euclid.
In 1966, US 421 was given one-way splits through central Greensboro: EB used Market St while WB used Gaston and Madison Ave (now Friendly Ave) west to a ramp that connects back to Market.
Between 1963-68, US 421 was rerouted in Siler City NC using Chatham-Fayetteville-2nd instead of Chatham-Beaver-2nd.
In 1968, US 421 was placed on newly constructed freeway at the south end of the Clinton Bypass, as it runs today, leaving behind bits of NC 24 and US 701, which was not added to the freeway at this time.

Also about 1968, US 421 was moved to I-74 between Exit 123 and Exit 132 outside Greensburg IN, leaving behind Old US 421.
In February 1969 (NCDOT), US 421 was given a bypass of Bear Creek, Bonlee, and Mt. Vernon Springs NC, leaving behind SR 1174 Old US 421.
In September 1969 (NCDOT), US 421 was placed on its current crossing of the Cape Fear River. 3rd St north of Wooster became unnumbered (though became US 421 Business for a time later).

Between 1965-69, US 421 was removed from Hazelwood St, Carolina Ave and Holston Valley Pike in eastern Bristol TN
In January 1970 (CTB), US 23-58-421 was placed on newly constructed road west of Gate City VA, leaving behind SR 870.
In February 1970 (CTB), US 23-58-421 was placed on a new alignment (partially over SR 646) between Clinchport and Duffield VA, leaving behind SR 871, SR 883 and a bit of VA 65.
In June 1970 (AASHO), US 421 was given one way alignments at its northern end in Michigan City IN. NB used 9th St east from Franklin, then north on Pine St to US 12. SB used Washington St south from US 12, then 9th St east to Franklin. Franklin St north of 9th St became unnumbered.
In November 1970 (AASHO), US 421 was rerouted to use I-465 around the east and north of Indianapolis. The old route through the city: Southeastern Ave; Washington St; N. West St; Martin Luther King Jr St/Michigan St.

In June 1971 (AASHO), US 421 was placed on new alignment east of Madison IN. The old route became IN 107 (today this is Main St; West St; Michigan St.
Between 1969-72, US 421 was removed from SR 1920 Quewiffle Rd through Taylors Bridge NC.
In March 1972 (NCDOT), US 421 was removed from Liberty Rd from Julian NC north to just short of I-85 (now I-40). This also placed US 421 on a larger NC 62 duplex.
In June 1972 (NCDOT), US 421 was given a bypass of Goldston NC, leaving behind SR 2333 Ralph Sipe Rd and more SR 1176 Main St.
In August 1972 (CTB), US 58-421 was placed on newly constructed road to avoid Pattonsville VA and run directly to US 23 Duffield. This left behind a bit of SR 638 and some SR 604.
Around 1972, US 421 was removed from KY 3480 south of Manchester KY.
In October 1973 (NCDOT), US 421 was removed from SR 1384 Forestwood Plank Rd plus abandoned routing east of Deep River.

Between 1973-76, US 421 was removed from the SR 1377 loop in westernmost Wilkes County NC.
In October 1977 (NCDOT), US 421 was put on mostly new construction (plus a bit of NC 87) to avoid Broadway NC, leaving behind some NC 42 plus SR 1579 Broadway Rd/Main St.
Also in October 1977 (AASHTO), US 421 was rerouted in Monticello IN to avoid the courthouse area. US 421 had been on one way alignments with NB using Main/Washington/Railroad to Broadway, while SB used Broadway to Main. The new route left Main at Harrison, then used Railroad to Broadway. Indiana tried to cancel this request in October 1977 but it was too late for the fall 1977 meeting. AASHTO was going to vote on the rescinding of the re-route but in April 1978 Indiana changed its mind and kept the new routing.
About 1978, US 421 was removed from KY 72/KY 413 north of Harlan KY.
Between 1977-80, US 421 was removed from the SR 1389 loop in westernmost Wilkes County NC.
In the early 1980s, US 421 was removed from Comprehensive Dr/abandoned/Skidmore Dr and Woodland Hills/abandoned in Grays Knob KY; also from KY 72 through Harlan KY
In December 1984 (NCDOT), US 421 was removed from Point Harbor Rd at the Cape Fear River.
In October 1984 (CTB), US 421 was moved onto this routing through Bristol VA: Commonwealth south then cut east on Goode, north on Piedmont, east on Cumberland, south on Martin Luther King Jr, then east on State to Pennsylvania Ave.
In 1985, US 421 was removed from Bristol Caverns Hwy east of Bristol (now TN 435)
In January 1988 (NCDOT), US 421 was removed from downtown Greensboro by using Wendover Ave from Market St north and east around to US 29 O Henry Blvd.

In October 1989 (KYTC), US 421 was rerouted in Frankfort KY to use the new twin bridges over the Kentucky River west out of Franfort. The old route is Broadway St/abandoned bridge/KY 1211.

In January 1991 (NCDOT), US 421 was given its freeway bypass of Siler City and Staley NC, leaving behind a bit of S. Chatham Blvd Ext plus more Old US 421.
In March 1994 (CTB), US 58-421 was put on its current routing over Wallen Ridge west of Stickleyville VA, leaving behind SR 915.
In May 1996 (NCDOT), US 421 was placed on today's I-40 around the south of Greensboro west, and on the now decommissioned I-40 Business bypassing Colfax and Friendship
In July 1997 (NCDOT), US 421 was removed from E. King St just east of NC 194 Boone.
In June 1999 (KYTC), US 421 replaced KY 876 on a bypass around the east of Richmond KY. The old route through town became US 421 Business, plus KY 3087 Old Lexington Rd.
About 2002, US 421 was substantially rebuilt between Morrill and Bighill KY. There are no surviving bypassed segments.
Also about 2002, US 421 was rerouted back through Lexington KY, replacing US 421 Business. The former bypass is now part of KY 4. The route through downtown was now on one way alignments, with NB using the original Main St alignment and SB using Vine St.

In September 2003 (NCDOT), US 421 was placed on newly constructed freeway between I-77 and US 601, leaving behind Rocky Branch Rd.
Also in September 2003, US 421 was put on new freeway in eastern Wilkes County from Exit 270 to Exit 276 near Antioch. This left behind Yadkin-Willkes Hwy.
About 2004, US 421 was placed on new alignment around Crummies and Cawood KY, leaving behind the lengthy KY 3001; also removed from two discontinuous Old US 421 stubs south of Harlan KY.
In December 2005 (NCDOT), US 421 was placed on new freeway from I-77 west to Exit 270, leaving behind Lydall Westex Rd and Yadkin-Wilkes Hwy.
Also in December 2005 (NCDOT), US 421 was placed on new construction from the S. New River east to Deep Gap, leaving behind a lengthy "Old US 421".
About 2008, US 421 was removed from Pennsylvania Ave and State St (became TN 473 for a time) in Bristol TN to instead use the new Anderson St bridge, then MLK Jr Dr north to Virginia.
Also about 2008, US 421 was removed from Cleo Ln and the original directional interchange where US 421 joined I-74 near Greensburg IN.
In November 2009 (NCDOT), US 421 was rerouted away from Greensboro entirely by being added to the new I-85 westward, then on I-73 northwest back around to I-40. Liberty Rd between new and old I-85 became SR 3762.

In 2011, US 421 was rerouted in Bristol VA to stay with US 11E-19 south into Tennessee, then east on Anderson Ave, leaving behind US 11-19 Truck; Martin Luther King Jr Blvd.

In February 2015, US 421 was placed on a bypass of Sanford NC around the east and north of town. Parts of this bypass were opened and designated as NC 87 a few years earlier. The old route through Sanford became US 421 Business.

In 2019 or 2020, US 421 was rerouted in Madison IN to use Harrison/Sering/Main coming off the Ohio River Bridge. Up to now US 421 used Harrison/2nd/Baltimore/Main.
Comments: US 421 is a major corridor really only in North Carolina.
Both I-685 and I-777 are planned to use a lot of the US 421 freeway alignments in North Carolina.
