Creation: Appeared in December 1926 as an original US Highway, replacing WV 52, running generally as it does now.
| Adjustments: Between 1950-59, US 11 was removed from CR 11/5, 11/17 Foster Rd plus CR 11/16 Foster Rd and CR 11/4 Encampment Rd all within Falling Waters. About 1976 (bridge date), US 11 was removed from CR 45/5 Eagle School Rd which used to connect to US 11 at CR 11/24 Warm Springs Ave north of Martinsburg. Improvements: Fully paved upon inception; no multilane segments Comment: One of the least changed US routes in West Virginia.
