Creation: Appeared in 1927 or 1928 as the 2nd TN 54, running from TN 59 (now Main St) Covington to TN 69 Paris. Covington to Brownsville was new state highway; Brownsville to Trenton was a renumbering of TN 41; Trenton to Greenfield was a renumbering of TN 5; Greenfield to Dresden was a renumbering of TN 42; Dresden to Paris was new state highway.

Adjustments: In 1930, TN 54 was put on its current routing from Alamo to Trenton. The old route used: Cherry St/Emerson Rd until it bends east; abandoned roadway northward; Malone Rd/Raleich Bird Rd; abandoned routing across the Middle Fork Deer River; Trenton Rd/Barber Rd/Old Alamo Rd, Layman Rd, a bit of Bill Edwards Rd; abandoned routing to TN 54 in the Frog Jump area; it appears the old route also used Lowery Ln Rd/TN 457/Poplar Corner Rd approaching Trenton.
Also in 1930, TN 54 was given its current routing from Trenton to Greenfield via Bradford. Originally TN 54 used Neal Kinsey Rd/Old Trenton-Dresden Rd/Lynn Point Rd/Hillsboro St
Also in 1930 it is likely when TN 54 was removed from Hearn Rd in Mt. Herman; Stalling rd east of Dresden; Jolly Rd and Central Rd near the Henry-Weakley Line
By 1934, TN 54 was removed from Old Dresden Hwy and Bucy Ln/Garland Ave west of Paris
In 1939 or 1940, TN 54 was extended as new state highway from Paris north through Puryear to Kentucky to meet KY 95. Note TN 54 never used the lengthy Old Paris-Murray Rd.
About 1963, TN 54 was rerouted in Trenton to cross US 45W Bus and use Armory Dr to the new US 45W Bypass north and east around central Trenton. The old route used today's High St to Eaton St to US 45W Bus and Halliburton St.

By 1970, TN 54 was on one-way splits east of the Covington Court House. WB used the original routing on Liberty Ave and EB used Pleasant and Maple
In 1985, TN 54 was extended from the Covington Court House west to US 51 as a replacement for TN 59 which was removed from the downtown area.
By 2008, the EB routing of TN 54 was removed from College St and uses Pleasant Ave throughout.

In 1925 or 1926, TN 54 was renumbered as TN 26.
Today the route is: Park Ave out of Lebanon; US 70 to Sparta except Taylor Rd loop at Cherry Valley, Main St through Watertown, Old State Hwy 26 east of Watertown, Old Alexandria Rd segments and Main St into Alexandria, High St out of Alexandria, Old Liberty Rd, legthy old Hwy 53 down to Dodd Store, Main St through Liberty and Dowelltown, Old Snow Hill Rd (3 segments), Main St through Smithville, Midway Rd through Midway, abandoned alignments to the west of 70 approaching Caney Fork River, a little underwater routing, Old State Route 26, Jack Hudson Rd, Ollie Billings/Howard Redmon Rds, Peeled Chestnut Ln, Old Smithville Hwy S, TN 136, Old Smithville Hwy N (cutoff by TN 111), Clarks Mill Dr into Sparta.
