Creation: Created about 1942 as new primary routing, replacing CR 31 from US 219 Peterstown to Ballard, then new construction to Red Sulphur Springs, then replaced Monroe CR 21, Summers CR 31 and CR 21 to end at WV 3 . This is the 2nd WV 12.
| Adjustments: About 1945, WV 12 north was extended east with WV 3 to Alderson, then replaced WV 54 north to US 60 Alta. By 1946, WV 12 south was extended south with US 219 through a tip of Virginia to Oakvale WV, then replaced CR 38 to end at US 19-21 near Bluefield. About 1968, WV 12 north was rerouted to accommodate I-64, leaving behind CR 12/7 which is cutoff by I-64 from CR 60/38. In 1973, WV 12 (along with US 460) was moved onto the new Corridor Q alignment leaving behind CR 460/6 Goodwins Chapel Rd. and the Ball Ave loop at the state line. In 1974 or 1975, WV 12 south was truncated to US 219 Peterstown with the Bluefield-Oakvale segment becoming WV 112.

WV 12 #1: Appeared 1922 as new primary routing running from WV 67 (now WV 10) Elmore north to WV 4 (now WV 210) Beckley.
In 1932, WV 12 was extended west with WV 10 to PIneville, then south as new primary routing to Welch, then replaced WV 64 through War to end at the Virginia State Line at Bishop. In 1938, WVDOH studied requesting replacing WV 12 from Elmore to Beckley with an unspecified US route number. It is unclear if they formally requested this or not but it obviously did not happen. In 1941, WV 12 was renumbered as part of WV 16. Note that WV 12 used CR 16/14 between Sophia and Crab Orchard; Neville St and Main St into Beckley
